Baptist Primary Care, Inc. is the region’s largest network of primary care physicians, with 40 office locations spanning five counties in northeast Florida and southeast Georgia.
Our roots in the community date back to the early 1990s, when a small group of respected physicians affiliated with Baptist Health of Northeast Florida. Incorporated as Baptist Primary Care in 2001, our network today includes more than 100 board-certified family physicians, internists and pediatricians.
Working as a team, our doctors, physician assistants, advanced registered nurse practitioners and staff provide high-quality care to our patients. Currently, we are working to complete the integration of Electronic Medical Record technology for advanced patient care and safety.
As part of Baptist Health, our patients benefit from access to the most preferred health system in Jacksonville, with more than 2,500 specialists and five nationally accredited hospitals.
Should our patients require hospitalization in a Baptist Health facility, our Hospitalist Program was designed to provide 24/7 oversight of your inpatient stay, acting as an extension of your primary care physician.
Board of Directors
Baptist Primary Care is a physician-led network of primary care physicians. The Board of Directors meet regularly and govern the organization. Perry G. Carlos, DO is currently serving as the Chairman of the Board and James W. Clower, III, MD is serving as the group medical director.

Perry G. Carlos, DO
Chairman of the Board
A Detroit native who received his Bachelor of Science and Master of Science degrees from Wayne State University in Detroit. He received his medical degree from the Chicago College of Osteopathic Medicine in 1974. After he was inducted into the Navy, Dr. Carlos completed his internship at Naval Hospital San Diego and his residency in family medicine at Naval Hospital Jacksonville.
Dr. Carlos is Board Certified in Family Practice and practices full time at Baptist Primary Care in Mandarin.
James W. Clower III, MD
Chief Medical Officer
Dr. Clower received his Bachelor of Arts from Tulane University in New Orleans. He received his medical degree from the University of Florida College of Medicine at Gainesville in 1981, and completed his residency in family practice at University of Florida/Alachua General Hospital in Gainesville. Dr. Clower is board certified in family practice medicine and has practiced on the westside of Jacksonville since 1984. He joined Baptist Primary Care in 2001 and sees patients at Baptist Primary Care on Lane Avenue.
