    From the Doctor

    I lead an active lifestyle, and I want my patients to do the same. I believe that healthy habits should be established at a young age, so I do my best to explain to parents the importance of exercise and nutritious diets, even for young children.

    I am originally from Mobile, Alabama. I went to college in Memphis and graduated with a degree in Zoology, then went on to study medicine in Birmingham and Charleston.

    I've lived here in Jacksonville for 11 years, and I love the easy access to the ocean. Sea kayaking is one of my favorite sports. I also enjoy hiking, mountain biking, snow skiing and traveling.

      Authorizes Baptist Primary Care to leave appointment, referral and test results information on your answering machine.

    • Medical Records Request or Release

      Initiates your request to have your medical records sent to another medical provider and/or to request your medical records from another provider be sent to your primary care doctor.

    • Health Care Status Authorization

      Authorizes Baptist Primary Care to release information concerning the status of your health care, including laboratory and radiology results and to discuss your treatment plan with an individual that you name.

    • Patient Registration and Insurance Information

      Captures your insurance information and authorizes Baptist Primary Care to prepare and submit your medical bill directly to your insurance company.
