    From the Doctor

    In my 35 years of caring for infants, children and adolescents, my single greatest focus has been on preventive care. I believe you can make a much bigger difference in the ultimate outcome when you start earlier in life.

    As I've watched my patients grow, my family has grown too. My wife and I have five children, 13 grandchildren and a golden retriever, and they keep us busy!

    In addition to spending time with my family, I also play an active role in the community. I serve on the board of directors for Hope Haven Children's Clinic and I am involved with the Special Olympics. I also enjoy playing tennis, duplicate bridge and golf.

  • • Authorization for Use of Answering Machines

      Authorizes Baptist Primary Care to leave appointment, referral and test results information on your answering machine.

    • Medical Records Request or Release

      Initiates your request to have your medical records sent to another medical provider and/or to request your medical records from another provider be sent to your primary care doctor.

    • Health Care Status Authorization

      Authorizes Baptist Primary Care to release information concerning the status of your health care, including laboratory and radiology results and to discuss your treatment plan with an individual that you name.

    • Patient Registration and Insurance Information

      Captures your insurance information and authorizes Baptist Primary Care to prepare and submit your medical bill directly to your insurance company.
