1. Find Your Doctor
By Convenient Location:
- Visit our Find a Doctor map.
- Click on a pin to find the doctors who practice at that location.
- Select a doctor to request an appointment.

- Select from Family Physician, Internist or Pediatrician.
Find a Doctor by Name:
- If you already know the doctor's name, please visit our Doctor Directory.
- Click on the name of the doctor.
2. Requesting an Appointment – New Patients
An office representative will call you within 1 business day to confirm your appointment date and time. You also can contact the office directly during business hours to schedule an appointment.
3. Preparation for Your First Visit:
From your doctor's page, you can print out and complete the new patient registration forms.
Confirm that your doctor accepts your insurance plan.
If you are experiencing any health issues, write down your symptoms in chronological order and any questions you would like to ask. If you have relevant medical records, please request copies or arrange to have them sent directly to your new doctor.
If you have any prescriptions or are taking any over the counter drugs, please prepare a list of the drugs that you’re taking and the dosage. Note any allergies or bad reactions that you have had to any medicines in the past.
4. What to Bring to Your Appointment
Please bring your new patient registration forms, a list of medications, allergies, immunizations, medical records, and your insurance card.
